FLSA STATUS:         Non-Exempt

REPORTS TO:         Director of Nursing and Transportation Coordinator

Health Center - Transportation Certified Nursing Assistant (CNA)  M-F 8-4 (Full-Time, Charlton Mass)  

POSITION SUMMARY :

The Transportation Certified Nursing Assistant promotes quality of life and a home environment by knowing the residents and from the framework of that knowing providing assistance with activities of daily living, and other direct care to residents as outlined in the interdisciplinary care plan, guided by the facility policies and under the direction of a licensed nurse.  POSITION DUTIES/ESSENTIAL FUNCTIONS:

  • Assist Health Center residents to and from appointments at the direction of the Transportation Coordinator

  • Participate on Nursing units when not transporting Residents

  • Recognize the autonomy and dignity of all residents.

  • Communicate a sense of caring, concern, and dignity for residents.

  • Understand how to place decision-making in the hands of the residents whenever possible and appropriate.

  • Make prompt and accurate judgments with regard to resident care and emergencies.

  • Work and communicate effectively as part of the unit team.

  • Serves on facility committees and work teams as assigned.

  • Participates in interdisciplinary care plan meetings.

  • Maintain all aspects of confidentiality regarding the residents and the facility.

  • Know and follow facility policies and procedures.

  • Use resources with consideration to cost.

  • Assist Staff with Resident Care, such as bathing, combing hair, brushing teeth, shaving,  dressing, ambulation, and toileting and laundry as needed. 14. Records and documents care provided to the resident 

  • Obtain routine lab samples.

  • Help residents with equipment for their treatment and care

  • Help residents to move, transfer from bed to chair, or transport them within the building or on the grounds.

  • Follow all infection control policies.

  • Observe the resident and surroundings and remove any potential hazards which could cause accidents.

  • Report to the nurse in charge all information and observations about the residents in their care

  • Assist as a backup scheduler for the nursing department

  • Actively engage Team Members on the nursing units, by phone and through digital means (such as the scheduling program) for the purpose of filing staffing vacancies.

  • Work together with nursing leadership to ensure compliance with the approved schedule and budget.

  • Coordinate with temporary staffing agencies to fill otherwise vacant scheduling needs within budget approval levels.  Maintain files and include temporary staff within the necessary databases for business purposes.

  • Updates and maintains the digital scheduling software with scheduling requirements and assigned staff.

  • May assist with other scheduling as needed.

      POSITION REQUIREMENTS:

  • Must possess a current Certified Nursing Assistant in accordance with the Massachusetts State regulations - required

  • Good judgment and decision making.

  • Communication skills including the ability to communicate with residents, families and team members.

  • Reliability including reporting to work as scheduled and on time, and completing assignments according to facility policy.

  • Patience, tact, and a willingness to take direction and to work as a team member

    PHYSICAL REQUIREMENTS/WORKING CONDITIONS: Primary work will be performed indoors in an office setting, may be exposed to the elements (heat, cold, rain, snow); walking and standing frequently; occasional lifting of up to 35 pounds.
